Healthy Families America/Smart Start Program
The Healthy Families America/Smart Start (CFF HFA/ Smart Start) Program helps expectant and new parents get their children off to a healthy start. The program strives to provide parents an opportunity to get the education and support they need at the time their baby is born, and until their child turns three.
Q:
Who is eligible to receive services?
CFF HFA/Smart Start targets women living in Delaware who are pregnant or have a newborn younger than three months old, and who need assistance with medical or behavioral health issues. A CFF HFA/Smart Start home visitor will determine final eligibility.
Some first-time moms may be eligible to participate in our
Nurse-Family Partnership program.

What services are provided during pregnancy?
Getting ready for a new baby can be both exciting and stressful. During your pregnancy, a CFF HFA/Smart Start home visitor will visit with you on a regular basis to give you the support you need to have a healthy pregnancy and prepare for the birth of your baby. What services are offered after the baby is born?
After the baby is born, your CFF HFA/Smart Start home visitor will continue to visit with you in your home regularly until your baby's 3rd birthday. Your home visitor will:
Help you learn to identify your baby's needs;
Share information on your child's development;
Share ideas on caring for babies, toddlers, and young children;
Make sure you and your child are connected with medical care;
Help you follow-up with recommended immunization schedules;
Link you with other resources in the community for help with job placement, identification of child care providers, etc.; and
Help you feel more empowered;
